
Modern Health Therapy Agreement
Contract for Therapy Date: April 2025

Purpose of the agreement is to:
- To provide a framework for safe therapeutic work.
- Confirming your identity and location and age.
At our first session I will ask you to confirm your location, name and age (if you are under 18 years
old) before embarking upon therapeutic work with you. - GP details/ emergency contacts
I may ask you for details of your GP as well as an emergency contact if there are urgent and serious
concerns about your welfare. - Structure
We will meet for 50 minutes sessions at intervals which are mutually agreed. We will meet via zoom. - Cancellations/ Rescheduling
Sessions will be booked through the Modern Health app at a mutually convenient time. If you need
to cancel or reschedule, please give me as much notice as possible.
If sessions are cancelled with less than 24 hours’ notice Modern Health treat this as a short notice
cancellation and a session will be deducted from your available sessions total.
I will only cancel session in urgent circumstances such as illness or emergencies and will contact you
as soon as possible. - Confidentiality, Privacy and Record Keeping
I will keep your sessions confidential. If there are serious concerns related to risk to you, I may need
to speak to someone else. This will be discussed with you in all circumstances unless the situation is
urgent. If a child’s safety is at risk, I have a legal responsibility to pass this information on.
Sometimes a situation requires more support than could be provided by a therapist in private
practice. In these circumstances if you wish I will try to redirect you to other appropriate sources of
support and care.
All therapists are required to have supervision as part of maintaining professional standards. Our
therapy work may be discussed in supervision with my supervisor. This is a confidential space and
details which would identify you will not be used.
I keep minimal written records about our sessions to remind me of our discussions and emergency
contact details. Notes are not legal documents. These are kept on a database called Writeupp which
is secure and confidential. Notes can be made available to you upon request and at the end of our
work together they will be retained for 7 years in line with the requirements of my professional body
and in case you contact me during this time. Following this the records of your sessions will be
destroyed. - Number of Sessions
Modern Health and your employer negotiate the number of sessions available to you through the
Modern Health app. Additional session need to be negotiated with them.
The first session is an opportunity to see if we can work together and if I am able to help. You can
change therapist by contacting Modern Health as any time, although I welcome you bringing any
difficulty to me first.
The number of sessions you have renew annually, at the end of a calendar year. - Fees
If you wish to purchase additional sessions this may be possible. Fees will be discussed when this is
requested. - Complaints
If you have concerns about your therapy, please raise it with me first. You can raise concerns with
me verbally or in writing. If you have a serious concern about the safety of my practice and you wish
to make a formal complaint to the professional body, then I am accountable to the Association of
Family Therapy and Systemic Practice which is part of UKCP. - Working electronically
You will need a confidential space in which to engage with your therapy – this means out of hearing
range of those not involved in the session.
The most updated version of Chrome is the best browser to use with the Modern Health website. At
times messages take time to come through the app and website. Following up any important
message with an email is a way to guarantee a message reaches me.
To ensure you get the most out of the session and to ensure that we are working safely I don’t do
therapy with people who are under the influence of alcohol or drugs so please attend your session
free from drugs or alcohol. You may wish to have a non-alcoholic drink with you for the session such
as a drink of water.

If we get cut off during our session, I will attempt to contact you by phone, email or message. If I am
unable to reach you then I will send a follow up email in order that we can reschedule. I will attempt
to contact you for 15 minutes but if that hasn’t been possible will stop at that point. If internet
access is a problem for any of us I can do individual session on the phone but this is difficult for
couple or family sessions so is not offered.
Under no circumstances will I make a recording of your session without your permission. I also
politely request that you do not make recordings of our sessions unless we have discussed this first.
In some situations online therapy may not be the best approach. If I think this is the case I will
discuss this with you. - Therapist emergency, illness or death. In the unlikely event of my serious illness or death, my Clinical Supervisor has access to a password-protected document which will allow him access to my notes. He will then contact you, or the Modern Health team will contact you to inform you of the situation. Please see the UKCP website for lists of alternative therapists.
